What is a Dental Implant?

A dental implant is a small titanium screw that serves as the replacement for the root portion of a missing natural tooth. Dental implants can be placed in either the upper or lower jaws. Due to the biocompatible properties of titanium, a dental implant fuses with the bone and becomes a good anchor for the replacement tooth. Dental implants can be used in solutions for replacing single or multiple missing teeth.

Many people who consider implants have removable, conventional dentures for lower and upper jaws, or have removable bridges. These people experience a significant improvement in their ability to chew food comfortably after their dental implant treatment.

The Dental Implant Procedure

The implant team usually includes either a periodontist or an oral and maxillofacial surgeon who implants the posts into the patient’s mouth, and a prosthodontist or general dentist who designs and fits the replacement teeth or permanent bridge. Dr. Timothy Tufankjian, DDS will implant, design and fit the replacement teeth or do restorative work on faulty crowns all in one office.

At the first appointment, we use an X-ray to determine if the jaw bone is adequate to hold the titanium posts. Models of the mouth are often made using a soft, pliable plastic compound. These models show where the gaps are located and provide measurements for a replacement tooth or a bridge. Treatment with dental implants is complicated and takes meticulous planning for best results. Today, the implants being placed often only require one surgical procedure using a local anesthetic and sedative.

Is Old Age a Problem?

All on 4-6 Dental Implant Procedure

In order for us to properly evaluate your case you will initially require a consultation and diagnostic appointment in order to best determine the treatment path necessary for your case. This includes determining whether or not your case will require extractions, gathering the appropriate information via X-rays, Panoramic scans and impressions and any other photos required. Directly after this consultation, you will be scheduled for implant surgery in which the new replacement teeth are fitted and adjusted all in one appointment, including any necessary extractions.

  • 4-6 titanium implants are placed in the jaw in such a way that the bone will envelop and secure them into place.
  • The implants that are placed in the back part of the jaw are angulated to achieve maximum advantage to the existing bone structure.
  • Once the implants are secure, the abutments will be attached so that the replacement teeth can be fastened and adjusted for maximum comfort and function giving you a brand new radiant smile.
  • Because only 4-6 implants are used in the procedure, the entire process is much less invasive and involves less surgical time than previous treatments.

What is a Dental Bridge?

Bridges span the space created when teeth are missing. A bridge may be recommended if you’re missing one or more teeth. A bridge is made up of two (or more) crowns for the teeth on either side of the gap with a false tooth or teeth in between. Dental bridges are supported by natural teeth or implants. Gaps left by missing teeth eventually cause the remaining teeth to rotate or shift into the empty spaces, resulting in a bad bite.

The imbalance caused by missing teeth can also lead to gum disease and temporomandibular joint (TMJ) disorders.

Non-Removable Implant Supported Bridges

When several teeth in a row are missing, they can be replaced by a bridge anchored to implants. An implant is not needed for each tooth being replaced. Three or four teeth can be replaced with just two dental implants for support. This is less expensive than placing an individual implant for each tooth being replaced.

Dental implants provide several advantages over other teeth replacement options.

  • In addition to looking and functioning like natural teeth, implant-supported bridges replace teeth without support from adjacent natural teeth.
  • Additionally dental implants are strong and can support the biting forces of several teeth when they are anchored together.
  • Because implant-supported bridges will replace some of your tooth roots, your bone is better preserved. With a fixed bridge or removable partial denture, the bone that previously surrounded the tooth root may begin to resorb (deteriorate). Dental implants integrate with your jawbone, helping to keep the bone healthy and intact.

Crown Supported Dental Bridge?

A bridge is made up of two (or more) crowns for the teeth on either side of the gap. These two or more anchoring teeth are called abutment teeth with a false tooth or teeth in between. Dental bridges are supported by natural teeth or implants.

  • A dental bridge consists of two crowns, which are attached and anchor the the replacement tooth in between.
  • These two crowns are cemented to the abutments – the natural teeth on either side of the empty space and act as anchors for the replacement tooth.

As with crowns, you have a choice of materials for bridges. Our dentist can help you decide which to use, based on the location of the missing tooth (or teeth), its function, aesthetic considerations and cost. Porcelain or ceramic bridges can be matched to the color of your natural teeth.

Bone Graft

Bone grafting can repair implant sites with inadequate bone structure due to previous extractions, gum disease or injuries. The bone is either obtained from a tissue bank or your own bone.

Causes for Jaw Bone Volume Shrinkage:

  • Periodontal Disease: Periodontal disease can affect and permanently damage the jaw bone that supports the teeth. Affected areas progressively worsen until the teeth become unstable.
  • Tooth Extraction: Studies have shown that patients who have experienced a tooth extraction subsequently lose 40-60% of the bone surrounding the extraction site during the following three years. Loss of bone results in what is called a “bone defect”.
  • Injuries and Infections: Dental injuries and other physical injuries resulting from a blow to the jaw can cause the bone to recede. Infections can also cause the jaw bone to recede in a similar way.

Sinus bone grafts are also performed to replace bone in the posterior upper jaw.

Sinus Lift (Graft)

Sinus Lift is a surgical method by which we can increase the amount bone tissue of the upper jaw, above the molars and premolars.

The maxillary sinuses are behind your cheeks and on top of the upper teeth and are like empty rooms that have nothing in them. Some of the roots of the natural upper teeth extend up into the maxillary sinuses. When these upper teeth are removed, there is often just a thin wall of bone separating the maxillary sinus and the mouth. Dental implants need bone to hold them in place. When the sinus wall is very thin, it is impossible to place dental implants in this bone.

To address this problem the dentist will enter the sinus from where the upper teeth used to be. The sinus membrane is then lifted upward and donor bone is inserted into the floor of the sinus. After healing, the bone becomes part of the patient’s jaw and dental implants can be inserted and stabilized in this new sinus bone.

If enough bone between the upper jaw ridge and the bottom of the sinus is available to stabilize the implant well, sinus augmentations and implant placement can sometimes be performed as a single procedure. If not enough bone is available, the Sinus Augmentation will have to be performed first, then the graft will have to mature for several months, depending upon the type of graft material used. Once the graft has matured, the implants can be placed.
